What is happening

Recurrence is not a failure of effort. It is information.

A decision that will not stay decided is telling you something specific. Either the cause was never established and the decision treated a symptom, or the decision was taken but never owned by anyone with the authority and the time to see it through.

The pattern is recognisable. Papers get longer while options get narrower. The same three arguments are rehearsed by the same three people. Executives arrive with a recommendation and a quiet hope it will not be reopened. Trustees sense that something is unresolved but have no independent basis on which to say what.

Meanwhile the cost accumulates somewhere else. In management time, in delayed investment, in staff who watch a question return for the third year and draw their own conclusions about whether anything here changes.

If the same decision comes back, it was never a decision. It was a postponement with minutes.

Why the usual responses do not hold

What is usually missing is not commitment or capability. It is an accountable judgement about the cause, made by someone with nothing to sell you afterwards, and a decision meeting designed to end in a choice rather than a further paper.

A decision with a name against it, and a way to know it worked.

The work ends in a facilitated session where the findings are presented to the people who can act on them, the trade-offs are made explicit, and the choice is taken in the room. What is written down afterwards records a decision, not a set of options.

You will have an independent account of the cause, an agreed route for what happens next, a named owner for each part of it, and the two or three measures that will tell you whether the question has actually been settled.
